Does Notion have a form builder?

Yes. Notion Forms is built in on every plan: type /form, add questions that map to database properties, and share the link. Logic needs Business.

Yes. Notion has a built-in form builder called Notion Forms, and every plan can use it: you type /form on a page (or add a Form view to an existing database), write questions, and share a link. Each question is a database property, so every response lands as a new row in that database. The catches are on the edges: conditional logic needs the Business plan, you can only build forms on desktop or web, and public forms are always anonymous.

How Notion Forms works

According to Notion’s help center, all members on all plans can create and use forms. There are two ways in:

  • From scratch: type /form on any page and pick Form. Notion creates a new database behind the form and adds a Respondent property automatically.
  • From an existing database: click the + next to your views and add a Form view. You need Full access to the database for this.

A form is its database. Each question maps to a property, and edits sync both ways by default: rename a question and the property is renamed; add a question and a new property appears. You can switch off Sync with property name if you want friendly question wording (“What’s your budget?”) over a short property name (“Budget”).

Notion’s own forms guide lists the question types as single-select, multi-select, text, date, person, files & media and number, among others. Per question you can mark it required, add a description, show choices as a list or dropdown, allow a long answer, and cap how many options someone can pick.

On the submit screen you can change the button color and text, write the confirmation message, and send each respondent an email copy of what they submitted.

Sharing: who can fill it out

The Who can fill out setting has three options: anyone in your workspace with the link, anyone on the web with the link, or No access to close the form. Two details trip people up:

  1. Web forms are always anonymous. You don’t get the respondent’s Notion identity, so if you need a name or email, make it a required question.
  2. Duplicated template forms report to the original creator. Duplicate someone else’s template form and, the help center warns, responses go to them. Build your own.

The guide also confirms that people outside your organization, including people with no Notion account, can fill out a web-shared form.

A real example

Say you run a small agency and want a client intake form. In a page called “Clients” you type /form, add questions for company name (text), service needed (single-select: Website, Branding, SEO), budget (number), deadline (date) and a brief upload (files & media). You set it to “anyone on the web with the link” and paste the link into your proposal email.

When a client submits, a new row appears in the Clients database with every answer in its own column. From there you can use Notion’s database automations, which the guide describes as triggering when a property is set to a value, to set an Assigned to person or post a Slack message when “Service needed” is SEO.

What you can’t do on a Free or Plus workspace: show the “SEO” follow-up questions only to clients who picked SEO. That’s conditional logic.

The limits that matter

Conditional logic is a Business feature. Notion’s March 2025 release notes added follow-up questions based on earlier answers and said the feature “is available on Business and Enterprise plans.” The help center adds that it only works on multiple-choice questions. On the pricing page, Business is listed at $20 per member per month, against $10 for Plus.

Branding. The Free plan gets “Basic forms.” Plus adds custom forms and the option to remove Notion branding from public forms.

File uploads follow your plan. On Free, files are “Up to 5 MB” each; paid plans allow a ~5GB max per file. A portfolio PDF or a short video can blow past that on Free.

Desktop and web only for building. You can’t create or edit a form in the mobile app. Respondents can fill it out on a phone.

Big response tables get slow. The help center notes that very high response volumes can slow loading, and you can’t export from a Form view; export from a Table view instead.

Admins can switch it off. Enterprise workspace owners can block forms from being shared to the web, which takes down live public forms.

When Notion Forms is enough, and when it isn’t

My take: if your form is a simple intake (a request queue, an internal bug report, a feedback box) and everyone answering is fine being anonymous, use Notion Forms. It’s free, it’s already in your workspace, and there’s no integration to break. Move only when you hit a specific wall: conditional logic on Free or Plus, large uploads on Free, or wanting more control over how the form looks. That’s when a separate form tool writing into a Notion database earns its place.

That’s the gap nDBForms fills. It builds a form from your existing Notion database, maps fields to properties, and writes each response as a new row. Respondents don’t need a Notion account. Conditional logic, multi-page forms and Google Drive uploads (up to 10 MB per file) are on its Pro plan, and logic works whatever Notion plan you’re on. The limitation: it’s another tool in the chain. Each response is also stored in nDBForms’ own database, and the Free plan stops at 3 forms and 100 responses a month. For one quick internal form, skip it. See the pricing for the full plan details.

If you want to see what a finished Notion-connected form looks like, these walk-throughs cover common builds: a customer feedback survey, a job application form and an event RSVP form.

FAQ

Is Notion’s form builder free?

Yes. Notion’s help center says all members on all plans can create and use forms, and the Free plan includes basic forms. Conditional logic and removing Notion branding from public forms need a paid plan.

Do people need a Notion account to fill out a Notion form?

No, if you share the form to anyone on the web with the link. Those responses are always anonymous. Workspace-only forms require the respondent to be in your workspace.

Does Notion Forms support conditional logic?

Yes, but only on the Business and Enterprise plans, and only on multiple-choice questions. Anyone can still build and share a form without it.

Can I build a Notion form on my phone?

No. Creating and customizing forms works on desktop and web only. People can fill out forms on mobile.

Where do Notion form responses go?

Each response becomes a new page in the database the form is attached to, shown by default in a table view called Responses.
