To create a form in Notion, type /form on any page and pick Form, or open an existing database, click + next to your views and choose Form. Edit the questions (each one becomes a database property), set who can fill it out under Share form, and copy the link. You need the desktop or web app to build it, and Full access if the form sits on an existing database.
What you need
- Notion on desktop or in a browser. Notion’s help center says creating and customizing forms works only on desktop and web, not mobile.
- Any plan. All members on all plans can create and use forms.
Full accessto the database if you’re adding a form to one that already exists.
Option A: a new form from scratch
- Open the page where the form should live.
- Type
/formand selectForm. - Give the form a title, and optionally a description, icon and cover.
- Add questions with the
+below the last question.
Notion creates a database to hold the answers and adds a Respondent property automatically to capture names.
Option B: a form on a database you already have
Use this if you already track something in Notion, like client requests or job applicants. Answers land next to your existing rows.
- Open the original database. Not a linked copy (more on that below).
- Click
+next to your existing views. On a narrow screen, open the dropdown next to the current view and selectAdd view. - Select
Form. - Select
Create {#} questions. Notion turns your properties into questions.
To record who submitted, add a Created by property to the database, as the help center suggests.
Set up the questions
Each question maps to a database property, and the two stay in sync by default. Rename a question and the property is renamed; add a question and a new property appears. To word a question differently from its property name, turn off Sync with property name.
From each question’s menu you can:
- mark it
Required - add a
Description - show choices as a list or a dropdown
- allow a
Long answeron text questions - cap the number of selections on multi-select, relation and people questions
The forms guide lists single-select, multi-select, text, date, person, files & media and number among the question types.
Under Form settings, the Submit screen lets you change the button’s color and text, write the confirmation title and body, and email respondents a copy of their answers.
Share it
Open Share form and pick who can fill it out:
- Anyone in the workspace with the link: for internal requests. Guests can respond only if they have access to the connected database.
- Anyone on the web with the link: for clients, applicants and customers. These people don’t need a Notion account. Web-link forms are always anonymous.
- No access: closes the form.
Copy the link and send it, or put it in an email signature or on your site.
A worked example: a content request form
Notion’s guide uses a content request form. A marketing team builds the form on their content calendar database with a “Content needed” select property. Someone in sales picks “Blog post” and submits.
Then an automation does the routing. From the form builder, select ••• → Automations. Set the trigger to “Content needed is set to Blog post” and the action to set Assigned to to the blog writer. Add a Slack notification if the team lives there. Now nobody has to watch the Responses view.
How to check it worked
- Click
Previewto see the form the way respondents will. - Submit one test response, from a private browser window if the form is public.
- Open the database. Responses go to a table view named
Responsesby default, with each question as a column. - Delete the test row.
What goes wrong
The Form option is missing. Notion’s guide says forms can only be added to the original database that holds the data. A linked view of a database that lives elsewhere won’t show it. Open the source database.
Responses go to someone else. The help center warns that duplicated template forms send responses to the original creator. If you copied a form from a template someone shared, rebuild it yourself.
Your answers are anonymous when you wanted names. That’s how web forms work. Add a required name or email question.
You can’t export the form. Form views don’t export. Export the questions and responses from a Table view.
The Responses view is slow. The help center notes very high response volumes can slow loading. Filter the view or archive old rows.
Follow-up questions don’t show conditionally. Conditional logic needs the Business or Enterprise plan, per the March 2025 release notes, and on the pricing page Business is $20 per member per month.
My take
I’d always use Option B. Building a form on a database you already have means the answers show up in the views, filters and automations you already use, and you decide the property names before the form exists. Starting from /form gives you a fresh database you’ll probably end up restructuring.
The real limitation is that a native form can’t be smarter than your plan. On Free or Plus there’s no conditional logic, and Free caps uploads at “Up to 5 MB” per file. If you hit those, nDBForms builds a form from the same Notion database, auto-generates fields from its properties and writes each answer as a row, with logic on its Pro plan regardless of your Notion plan. You can start from one of the form templates.
For finished examples of database-backed forms, see the walk-throughs for an IT support ticket form and a job application form. If you’re still deciding whether Notion’s own builder is enough, read what Notion’s form builder can and can’t do.
FAQ
Why don’t I see the Form option in my Notion database?
You are probably in a linked view. Forms can only be added to the original database that holds the data, so open the source database and add the Form view there. You also need Full access to that database.
Can I create a Notion form on my phone?
No. Building and customizing forms only works on desktop and web. Respondents can fill out the form on mobile.
How do I see who filled out my Notion form?
Web-shared forms are always anonymous, so add a required name or email question. For workspace forms built on an existing database, add a Created by property to capture who submitted.
Can I use a Notion form template someone shared with me?
Not safely. Notion warns that duplicated template forms send responses to the original creator. Rebuild the form from scratch in your own workspace.
